Please contact us for a sole source letter/affidavit, W-9, CIQ, or other required vendor forms (click here to download a sole source affidavit).    Information such as district vendor numbers, district bids, and purchasing cooperative contracts (including statewide bids) are also available upon request.  DynaStudy is proud to be HUB certified (Historically Underutilized Business) by the Texas Building and Procurement Commission.
